To ensure the safety, health, and well-being of every child in our care, we require a set of important forms to be completed and submitted. Below, you’ll find downloadable PDFs including the Universal Health Record, Medication Form, Babysitting Policy, Food Allergy Action Plan, Child Information Card, and Media Consent Form.

This is the required health form for all students, mandated by the State of New Jersey. It ensures your child is healthy and safe to participate in our programs. This record must be signed by your pediatrician and confirm your child has had a physical exam within the last 12 months, along with an up-to-date immunization record.
If your child needs our staff to administer any medication—either prescription or over-the-counter—during school hours, this form is required. It gives us the specific authorization and instructions to ensure your child receives the correct medication safely. For safety and compliance, all medication must be in its original container with the prescription label on it.
This form is required if you hire one of our teachers to babysit your child outside of school hours. MMO Programs does not support this private arrangement. By signing, you confirm that the teacher is your employee (not MMO’s), and the school is not responsible for any issues that happen during that time.
If your child has any food allergies, we require a completed Food Allergy Action Plan form, signed by both a parent/guardian and your child’s healthcare provider. This form provides critical information on allergy triggers, symptoms, and emergency treatment instructions to help our staff respond quickly and appropriately.
To help us provide the best possible care, we ask all families to complete a Child Information Form. This form gives us important details about your child’s emergency contacts, and any special instructions or preferences. The more we know, the better we can support your child’s comfort, safety, and growth.
We love capturing special moments of learning, creativity, and fun! With your permission, we may take photos or videos of your child for use in classroom projects, newsletters, or our center’s website and social media. Please complete the Media Consent Form to let us know whether or not you grant permission for your child to be included. Your preferences will always be respected.

Returning customers will log in to your existing account and add your child for this school year; NEW users will need to create an account. (Click on the “How It Works” tab on our home page to follow step by step directions to create an account.)
You may choose to participate every day or any individual days during the month. Orders must be placed three days in advance of the day you would like to receive lunch service. Please keep this in mind when you are placing your orders. We encourage you to order for the whole month to avoid missing days. If your needs change last minute, just call us, we will be happy to add your order!!”
